13 yr old with fever and shortness of breath

Here is a case i have seen:

13 years old boy came to casuality with chief complaints of fever 4 days back and complaints of shortness of breath 2 days back

Fever is intermittent type, low grade in nature with no diurnal variation and was not associated with chills or rigors . For which he was taken to a local hospital and fever subsided with medication and later he developed shortness of breath after 2 days which was Grade 2 on ordinary activity and was taken again to a local hospital and was advised for corona testing hrct and rtpcr which came out negative  .he Complains of loss of appetite and constipation. 

No complains of vomitings, cough, cold, dysuria, loose stools, chest pain, burning sensation in chest, palpitations ands pallor. 

No similar complaints in the past history. 

Not a k/c/o hypertension, diabetes, epilepsy, asthma, tuberculosis or any other systemic illness. 

No h/o any past surgeries. 

On examination:

Patient was conscious coherent and cooperative.

General physical examination 

No pallor,icterus,clubbing,lymphadenopathy, pedal edema

Vitals: 

Temperature: 98.6 F

Pulse : 82 bpm

Respiratort rate: 18 cpm

Blood pressure: 100/80 mm hg

Spo2: 98% at room air

CVS: S1, S2 heard, no murmurs

Respiratory system: Bilateral air entry present, normal vesicular breath sounds heard, no added sounds heard

Per abdomen: soft, non tender, bowel sounds heard

CNS : GCS: E4V5M6 

           no abnormalities detected
